Right side up.  If I was worried about the stitching, I'd fold 2x so no stitching is shown on top.

My mom taught me to put the top sheet on upside down, so when it's folded over, yada yada.

But as an adult, I don't.  Top sheet goes on right side up, and I don't fold it down. 

Really I think this question is pointless, because it's all covered by a quilt or comforter or whatever, and when you take the comforter down you're just going to sleep in the dark anyway.

 

TP overhand, unless you have pets or toddlers who will mess with it.
