That's a pretty good one too. I've seen people connect Radio Shack speaker wire to a 480V 600 amp panel then put the cover back on the box so it pinched the insulation on the wire, aluminum foil wrapped around conduits to 'insulate' it from a heat source, steam coming out of underground conduits because the insulation got nicked by careless installation and the leaking current was boiling the water that seeped into the conduit, a panel installed so you had to stand in a mop sink to open it... On one job a contractor installed a 200 amp switch, but so close to other equipment that the cover wouldn't open all the way. That's a Code violation. He 'fixed' by cutting the metal cover with a jigsaw, then putting the two pieces back together with little brass hardware store hinges so the cover could fold. Guess what - Code violation number 2, because that voids the UL listing on the switch.
