The belt is actually not bad at all once you get used to it. I don't like to run with a lot of crap on me - I carry enough weight as it is. But hydration is a necessary thing.
The running belt goes over your shirt. You want to make sure that it doesn't bounce too much because it will cause chafing. You definitely want a layer of clothing between you and the hydration belt to prevent this. The insidious thing is that you may never know you are chafing until it is too late - when the run is done and you are standing in the shower and the water hits the chafed spot. Holy hell that hurts.
